Somebody should be concerned about Ifrit's health, she'd probably die within a week with no treatment if her Infection wasn't on hold aboard the train... but she don't know that.

Ifrit's probably not very up front about having a VOICE IN HER HEAD telling her to KILL EVERYBODY, though. Just like, it's not very kosher. And usually ends with people calling her a monster and stuff.
